For members on most GuideStone comprehensive plans, your vision benefit covers one annual eye exam per covered family member. The coverage does not include the cost of glasses or contact lenses. You must use an in-network provider to receive this benefit. The vision benefit is not available for GuideStone’s Value Health or Secure Health plans.

Benefits include weight management programs, massage therapy, acupuncture, fitness club memberships and vision discounts on contacts, glasses and frames. These benefits are available to enrolled members and dependents only. Programs and benefits are not available in all states and are not available with international plans.
For members with life insurance, personalized professional counseling service includes one-on-one financial counseling, a written personal financial plan and access to Ceridian-licensed counselors for your survivors in the event of your death or the diagnosis of a terminal illness.
For members with life insurance, terminally ill members with a life expectancy of 12 months or less can receive up to 75% of the death benefit ($250,000 maximum) prior to death.
For members with life insurance, you and your dependents can continue life coverage directly through Unum® if you leave your employer or otherwise lose eligibility.
For members with life insurance, no underwriting is required to add a dependent child to your term life coverage within 60 days of the child’s birth, adoption or placement for adoption.
For members with Accidental Death & Dismemberment (AD&D) insurance, there is an additional education benefit of 6% of the full amount of the AD&D benefit, up to $6,000 a year for up to four years. Your plan booklet provides requirements and details about this benefit.
To encourage members with disability insurance to return to work as soon as they become physically able, individuals receive an additional benefit for participating in a rehabilitation program. Details are provided in your disability plan booklets.
If a member with long-term disability insurance dies after receiving disability benefits for 180 or more consecutive days, their survivor will receive a lump sum payment of three times the member's last month’s gross disability benefit.
