OTC (Over-the-Counter) COVID Test Reimbursement Requirements

How do I know if my purchase is eligible for reimbursement?
  • Test must be purchased on or after January 15th, 2022, to be eligible for reimbursement.
  • The test you purchased is listed on the Food and Drug Administration Approved Test List.
  • You were exposed to someone with COVID-19 or you have symptoms.
  • You must provide documentation of the amount you paid — such as a receipt.
  • You must not submit more than eight test receipts per member per month. 
Where can I find an FDA Approved COVID-19 test?
  • Starting January 19, 2022, you can request free rapid antigen tests (without shipping fees) from the federal government.
  • Rapid antigen home tests may also be available at your local drugstore or online.
How many tests are eligible for reimbursement?
  • Eight (8) tests per month (or every 30 days) is the maximum number of tests that are eligible for reimbursement.
